The Scientific Information System (SIS_eAwards) enables individuals to submit applications for funding electronically. It also enables certain authorized individuals to support, approve, monitor, administer and/or evaluate such applications while respecting the confidential nature of the information provided.

All information entered into this system will be managed and protected in accordance with applicable federal laws and policies as set out below, and in accordance with these Terms of Use.

a) CONFIDENTIALITY, SECURITY, AND USE AND DISCLOSURE OF PERSONAL INFORMATION

Information provided will be transmitted in an encrypted file format and stored on a secure server.

Information collected using these web forms will be treated in strict confidence. In addition to the uses and disclosures outlined below it may also be used to identify and authenticate you when using the help desk or in the event that you forget your system account or password. Certain data requested at registration, such as your mother's maiden name and your date of birth, will be used only for authentication purposes.

b) CONDITIONS OF AGREEMENT

I declare and attest that:

1. I am an applicant, co-applicant, institutional administrator or reviewer for an application submitted to the agency.

2. All the information I will provide will be accurate and complete. I agree to inform the agency immediately of any change to submitted information.

3. I agree to abide by the strictest confidentiality with respect to my password. I also agree that I am fully responsible for the security of my password and that I will take all reasonable precautions to avoid disclosure to unauthorized individuals. I will notify the agency immediately if I suspect that the security of my system account or password logon information has been breached, or if I suspect that access to the SIS_eAwards has been or may be used unlawfully or without my consent.

4. Data collected in the electronic submission platform will be used for management purposes. Some of the project data (the name of the applicant, address of laboratory, the title of the project, its english lay summary (5 lines), the duration of the project and the AFM funding amount) will be the subject of automatic data processing by the Association Françaisecontre les Myopathies. A public web database will include these data to provide the general public with information regarding the progress of fundamental and clinical research related to neuromuscular diseases. Pursuant to the French law “Informatique et Libertés” of 6 Jan. 1978, you have the right of access to and the right to rectify your personal retained data. In case you wish to exercise these rights, please address requests to the “Secrétariat Permanent du ConseilScientifique” of AFMTELETHON (spcsafm@afm.genethon.fr). You have also the right to object, on legitimate grounds, to the processing of data related to you.

5. I understand that no changes will be made by the agency directly in any application or CCV submitted and that the CCV information received by the agency will be retained in a secure manner for my current and future use with the SIS_eAwards. It is my responsibility to keep my CCV updated.

6. I agree that the agency, in its sole discretion, is not required to accept any incomplete, inappropriate, inaccurate or late applications and that the agency, its staff, reviewers and funding partners will not be responsible for any incomplete, inappropriate, inaccurate or late applications. The agency may, where appropriate, reclassify or reject applications.

7. I will not use or disclose any information that I obtain or view in connection with my permitted access to the SIS_eAwards except for the purposes of my participation in the agency process.

8. I warrant that I will not use the SIS_eAwards for any purpose that breaches confidentiality or privacy of others, is unlawful or prohibited by this subscription agreement or that in any way could damage or impair the system or interfere with any other party's use of the system. I will not obtain, or attempt to obtain, any materials or information through the SIS_eAwards that I am not intended and authorized to access. NOTE: System activity is monitored. Unauthorized users risk prosecution.

9. The agency reserves the right, in its sole discretion, to terminate or restrict my access to the SIS_eAwards and the related services or any portion thereof at any time, without notice.

10. If I wish to close my SIS_eAwards account, I will contact the agency Help Desk (spcsafm@afm.genethon.fr).

When I select "I accept" below, I confirm that I am in full acceptance of these SIS_eAwards Terms of Use and that this electronic acceptance is equivalent to my handwritten signature. I recognize and accept that this agreement applies in full to all applications, documents and communications I may submit, view or obtain electronically or otherwise relating to the SIS_eAwards and the operations of the agency.
